What sets Garden State Eldercare apart from other providers? The answer is simple: we are a small,
local agency offering highly personalized attention and service – 24 hours a day, seven days a week. Following an initial complimentary consultation, our approach consists of three elements:
Assessment
A Registered Nurse and a Social Worker meet with the client/family to conduct a thorough
evaluation, or assessment. A plan of care is then developed in collaboration with the client’s
physician.
Assistance
Each plan of care, tailored to meet individual needs, can include:
Nursing Services
- Medication management, to insure clients take all medications in the proper dosages
- Monitoring of vital signs
- Supporting clients and families with education and tools
Certified Hourly/Live-in Home Health Aide Services
Our dedicated caregivers provide caring companionship and assistance with:
- Bathing, grooming and dressing
- Light housekeeping, including meal preparation, laundry, homemaking tasks and errands
- Medication reminders
Note: Home health aides are routinely supervised by a Registered Nurse.
Advocacy
The elderly and their families often need guidance in navigating the health care system
and locating the providers they need. Our geriatric care management service, provided by a social worker or registered nurse, provides assistance by:
- Evaluating needs
- Identifying available community resources and programs
- Coordinating services with physicians and other professionals
- Assisting with long term care arrangements/placement
